HealthCARE Express is designed to provide you convenient, affordable healthcare where you can feel great about the care you received.
NO INSURANCE, NO PROBLEM
HealthCARE Express gladly accepts patients without insurance. Our cash pay prices start at $175 for office visits. Additional charges will be applied for lab tests, vaccines, medications, supplies, complex procedures, tests, prescriptions, and x-ray services as deemed necessary by the medical provider. Posted prices include discounts for cash or credit card payments at time of service. Patient should report discount on any claim to a third-party payer.
PAYMENT OPTIONS
HealthCARE Express accepts cash, personal checks, Visa, MasterCard, and Discover credit cards. Payment is due at the time of service.
INSURANCE GLADLY ACCEPTED
HealthCARE Express accepts most major types of insurance including Medicaid, Medicare, SoonerCare, Tricare, TriWest and workers' comp insurance. HealthCARE Express is billed as an urgent care center to your insurance company. This means your co-pay may be cheaper than a visit to the emergency room, but may be higher than a visit to your primary care doctor. Please check your insurance coverage to find out if your co-pay will be different.

MEDICAL RECORDS REQUEST
To access or receive copies of your medical record, or to authorize HealthCARE Express to release your medical information to another facility or individual, you must submit a request with a signed authorization form to the Medical Records Department at HealthCARE Express.
Cost for Medical Records
We charge a fee for medical records to cover the costs of copying, mailing, and/or other supplies associated with your request. Upon receiving your request, we will inform you of the fee with an invoice.
Initial Medical Record Fee: $7.50
25 pages @ $1.00 per page up to 25 pages: $25.00
Additional pages: $0.50 per page from 26-500 pgs
Postage Expense: Rates vary based on size of medical record
Affidavit/Notary Fee: $15.00
X-Rays or Diagnostic Imaging: $8.00
